Schools of Choice Application

2025-2026

 

Students applying to the Ferndale Public Schools, Schools of Choice program must meet district Schools of Choice enrollment requirements. Applicants will be notified of their status and acceptance by the Office of Enrollment. According to state law, they must enroll and attend school by the end of the first week of school. 


I understand my child must be enrolled and attending by the end of the first week of school. All information on this form is true to the best of my knowledge, and I understand that parents/guardians of students enrolled through the Schools of Choice program are responsible for providing transportation for the student from home to school, see Ferndale Board of Education Policy 5111 - Eligibility of Resident/Nonresident Students.

 

As the parent/legal guardian, I understand that my student can be denied or disenrolled for inaccurate or falsified information reported on this application.
