Print or save a copy of the Enrollment Checklist (documents which must accompany every student enrollment in order to process)

Please complete the sections by following the instructions provided at each step

NOTE! Completing this on-line pre-enrollment form does not enroll your child in the Ferndale School District.
This pre-enrollment data will be kept for 60-days only.
Please make sure to complete the pre-enrollment in a timely manner in order to avoid the data from being deleted.
You can log back in and upload any missing documents. Please make sure they are legible if taking pictures.
Please do not send the documents to the enroll email address. They must be uploaded during pre-enrollment.
If you are missing documents we cannot process your child's enrollment.

If you uploaded all required documents, then there is no need to bring the required documentation
to the Enrollment Office. To make the process quicker please upload all required documents.
